Pelton's Community Adaptive Skills Team (CAST) provides home and community-based (HCBS) personal care and habilitative services to individuals in mental health recovery.
Services are provided in accordance to the goals, objectives, action steps, and frequency as outlined in an individual's person-centered plan. Services are trauma-informed, consistent, and staff forge strong professional supportive relationships with services recipients.
CAST services are Medicaid-funded services designed to help adults (aged 21 and over) with long-term mental health challenges live independently in their homes and communities rather than in institutional settings like hospitals or residential treatment programs.

HCBS services are administered by the Oregon Health Authority through a state plan option (1915(i)).

Individuals in mental health recovery who are seeking greater autonomy and self-direction to achieve the greatest independence possible through support services that uphold principles of dignity and respect.

Services are tailored to each individual's needs through a person-centered service plan.
Types of Services Offered:
Community-Based Integrated Supports: Helps individuals develop social skills and improve community living.
Transition Services: Supports the move from a facility to a community-based home.
Psychosocial Rehabilitation: Aids in acquiring or regaining skills necessary for living and working in the community.
In-Home Personal Care: Offers assistance with activities of daily living (e.g., bathing, dressing, shopping, and medication management).
Residential Rehabilitation: Provides support and training at home to enhance independent living skills.
Home-Based Habilitation: Focuses on developing skills like navigating the community, financial management, and self-care.
Housing Support Services: Assists in finding and maintaining suitable housing and addresses challenges along the way.
Pest Eradication Services: Intended to remove pests (currently not available).
Community Transportation: Facilitates travel to appointments and community events.
Home-Delivered Meals: Delivers meals to those who are homebound (currently not available).
